As a Care Coordinator (Permanent Full-Time, Immediate start, Location: Timmins), you will assess patient care needs, determine eligibility, provide access and referrals to community services, and engage with patients, caregivers, and healthcare practitioners.

Whether working in our office, in the community, or in a healthcare facility, you will play a key role in providing connected, accessible, patient-centered care, supported by our collaborative team of over 9,000 healthcare and other professionals.

As a valued team member, your mission will be to help our patients be healthier at home, while benefiting from our support for professional growth, personal wellness, and work-life balance.

What will you do?

  • Assess care needs and determine eligibility in collaboration with patients and families
  • Link patients with service providers
  • Coordinate and monitor care plan delivery
  • Establish helping relationships with patients and families
  • Balance patient needs and preferences with available resources, respecting their values

What must you have?

  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • Recent experience in community health or a related field
  • Knowledge of the healthcare system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management, prioritization, and organizational skills; ability to work independently and in a multidisciplinary team
  • Ability to accurately complete documentation, reports, and forms
  • Valid driver’s license and access to a reliable vehicle
  • Proficiency in English and French, both oral and written, is essential
  • Mandatory COVID-19 vaccination proof prior to start date

What would give you an edge?

  • Experience working with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics)
  • Case management or recent community experience
  • Ability to speak French or another second language
