Job summary

Ontario Health atHome is seeking a Care Coordinator (RN, PT, OT or SW) to assess patient needs, determine eligibility, and connect patients with community services. You will coordinate and monitor care plans, engaging with patients, caregivers and a multidisciplinary team.

The role offers full-time/temporary status with immediate start (through Sept 30, 2027) in North Bay or Sault Ste. Marie, with a strong emphasis on patient-centered home care and access to supports for professional growth.

Qualifications

  • Must have active registration with respective Ontario Colleges (Nursing/Physiotherapy/OT/Social Work).
  • Recent experience in community health or related field preferred.
  • Knowledge of Ontario health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment, problem-solving and decision-making skills.

Responsibilities

  • Assess care needs and determine eligibility for services; develop care plans.
  • Link patients with service providers and coordinate care delivery.
  • Monitor and adjust care plans; maintain documentation and reports.
  • Build rapport with patients and families; respect patient values and preferences.
  • Balance patient needs with available resources; work independently and with team.

As a Care Coordinator (Status: Full-Time,Temporary, Start date: Immediately to September 30, 2027; Location: North Bay or Sault Ste. Marie) , you will assess and determine patient care needs and eligibility, provideaccess and referrals to community services, and engage with patients,caregivers and other health care practitioners.

What will you do?

  • In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
  • Linkpatients with service providers
  • Coordinate and monitor care plan delivery
  • Establish a helping relationship with patients and their families
  • Balance patient needs and choices with available resources,ensuring patients’ values and preferences are respected
  • Initial assignment Community Intake, with possible assignment changeswithin the occupation based on operational needs

  • Collegeof Physiotherapists of Ontario
  • Collegeof Occupational Therapists of Ontario
  • OntarioCollege of Social Workers and Social Service Workers
  • Recentexperience in community health or a related field
  • Knowledgeof the health care delivery system and community resources
  • Excellentinterpersonal, communication, assessment, problem-solving anddecision-making skills
  • Effectivetime management, prioritization and organizational skills, with theability to work independently and co-operatively in a busy multidisciplinaryenvironment
  • Establishedability to accurately complete required documentation, reports and forms
  • Avalid driver’s license and access to a reliable vehicle
  • Advancedoral and written proficiency in English is essential
  • We have amandatory COVID-19 vaccination policy. As a condition of employment, allemployees are required to submit proof of COVID-19 vaccination statusprior to start date.

  • Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
  • Casemanagement experience or recent related community experience
  • Abilityto speak French or another second language

Salary: Hourly wage of $41.29 to $51.13

Terms of Employment: Temporary, Full-Time
