Job description

Are you an experienced Registered Nurse, Physiotherapist, Occupational Therapist, Social Worker, Dietitian, or Speech Language Pathologist seeking a rewarding career that cares for others? You’re in the right place.

As a Care Coordinator, you will assess patient care needs, determine eligibility, provide access and referrals to community services, and collaborate with patients, caregivers, and healthcare practitioners. Whether working in our office, community, or healthcare facility, you will play a key role in delivering connected, accessible, patient-centered care, supported by a collaborative team of over 8,900 regulated healthcare and other professionals. Projected start date: June 16, 2025, with 7 weeks of mandatory full-time orientation.

Note: You will be considered for all branches of interest within HNHB; however, we ask that you apply to only one branch location.

What will you do?
  1. Assess care needs and determine eligibility in collaboration with patients and families.
  2. Link patients with appropriate service providers.
  3. Coordinate and monitor care plan delivery.
  4. Establish trusting relationships with patients and families.
  5. Balance patient needs and preferences with available resources, respecting their values.
What must you have?
  • Membership, in good standing, with the applicable regulatory body:
  • College of Nurses of Ontario
  • College of Physiotherapists of Ontario
  • College of Occupational Therapists of Ontario
  • Ontario College of Social Workers and Social Service Workers
  • College of Audiologists and Speech-Language Pathologists of Ontario
  • College of Dietitians of Ontario
  • At least 2 years of recent experience in community health or a related field
  • Knowledge of healthcare delivery system and community resources
  • Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills
  • Effective time management and organizational skills; ability to work independently and in a team
  • Accurate documentation skills
  • Valid driver’s license and access to reliable transportation
  • Proficiency in Windows environment
  • Ability to provide a vulnerable sector background check

Note: We have a mandatory COVID-19 vaccination policy. Proof of vaccination is required before start date.

What would give you the edge?
  • Experience with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics)
  • Case management or recent community experience
  • Ability to speak French or another second language
Job Details
  • Position Length: Ongoing
  • Wage Range: $45.35 - $48.87 per hour
  • Temporary Full-Time
Hours of Work

Shifts available: Sunday to Saturday, various times including 8:30am - 4:30pm, 10:00am – 6:00pm, 1:00pm – 9:00pm, and 9am - 8pm.

Availability Requirements

Part-time B employees must:

  • Be available for at least six shifts in a two-week period, including at least one Monday or Friday, from 8:30am to 9:00pm.
  • Work one weekend out of three.
  • Be available for five paid holidays annually, including Christmas and New Year’s Day, on a rotating basis.
  • Be available for 46 weeks per year.
  • Limit time off during summer to three weeks.
  • Ensure no more than 50% of PT B employees in a branch are unavailable in any month, with conflicts resolved by seniority.
  • Submit availability for the following month on the first day of each month, with special deadlines for July and August.
