Job Description

As a Bilingual Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers, and other healthcare practitioners.

What will you do? Under the general direction of the Manager, Home and Community Care, the Care Coordinator is responsible for managing a caseload of diverse patients with various health concerns. The Care Coordinator will assess patient needs via phone, develop, implement, and update care plans based on health and psychosocial needs, and accommodate patient and caregiver wishes where possible. They will also determine eligibility for Ontario Health atHome Champlain-approved services and refer patients to community resources.

As part of the Intake Care Team, the Care Coordinator will assess and develop individual care plans over the phone, utilizing a multidisciplinary approach to achieve health outcomes. This is an in-office, non-visiting role where referrals are prioritized based on complexity, urgency, and needs, following Ontario Health guidelines.

Primary Responsibilities:

  • Complete comprehensive patient assessments using validated tools, considering clinical, physical, psychosocial, and caregiver needs.
  • Assess understanding and learning needs related to diagnosis, treatment, resources, and coping mechanisms.
  • Assist patients and caregivers in setting goals and developing individualized care plans.
  • Coordinate delivery of home and community care services.
  • Manage vendor and resource utilization for medical equipment, supplies, and services.
  • Facilitate communication among the healthcare team to maximize patient outcomes.
  • Implement care plans through contracted providers.
  • Provide regular reports to physicians, service providers, patients, and caregivers.
  • Document case details per Ontario Health policies.

Team/Department: Intake

Hours of work: Rotating shifts of 8 or 12 hours, between 8 am and 8 pm, 7 days/week, totaling 70 hours biweekly (Note: 12-hour shifts until June 7, 2025).

FTE: 1.0

Starting Salary: $44,480 to $47,517

Must Have:

  • Registered Nurse (BScN or diploma) with registration in Ontario.
  • Degree or diploma in Physiotherapy, Occupational Therapy, Social Work, Speech Therapy, or Dietetics with registration in respective colleges.
  • Related professional experience, preferably community-based, typically over two years.

Preferred Skills:

  • Experience working with diverse patient groups and managing sensitive conversations.
  • Ability to work independently and in teams.
  • Excellent communication, facilitation, diplomacy, and relationship-building skills.
  • Proficiency in computer software and Microsoft Office.
  • Valid driver’s license and access to a vehicle.
  • Fluency in English and French.
  • Recent Vulnerable Sector Check.
