Are you an experienced registered nurse (BScN), physiotherapist, occupational therapist, social worker, or speech language pathologist seeking a rewarding career that cares for others? You’re in the right place.

As a Bilingual (French) Care Coordinator, you will assess patient needs, determine eligibility, provide access and referrals to community services, and engage with patients, caregivers, and healthcare practitioners.

Whether working in our office, in the community, or at a healthcare facility, you will play a key role in delivering connected, accessible, patient-centered care, supported by a collaborative team of over 9,000 healthcare professionals.

As a valued team member, your mission will be to help patients stay healthier at home, with support for your professional growth, personal wellness, and work-life balance.

What will you do?
  1. Assess care needs and develop individual care and service plans in collaboration with patients and families.
  2. Link patients with appropriate service providers.
  3. Coordinate and monitor care plan delivery.
  4. Establish trusting relationships with patients and their families.
  5. Balance patient needs and preferences with available resources, respecting their values.
What must you have?
  1. Proficiency in both written and verbal French.
  2. Membership, in good standing, with the applicable regulatory body (e.g., College of Nurses of Ontario, College of Physiotherapists of Ontario, etc.).
  3. At least 2 years of recent experience in community health or a related field.
  4. Knowledge of healthcare delivery systems and community resources.
  5. Excellent interpersonal, communication, assessment, problem-solving, and decision-making skills.
  6. Strong time management, organizational skills, and ability to work independently and in teams.
  7. Ability to accurately complete documentation, reports, and forms.
  8. A valid driver’s license and access to a reliable vehicle.
  9. Proficiency in Windows environment.
  10. Mandatory COVID-19 vaccination, with proof required prior to start.

  1. Experience with diverse patient groups (multicultural, homeless, palliative, ABI, mental health, geriatrics, pediatrics).
  2. Case management or recent community experience.
  3. Proficiency in French (written and verbal).
