Job summary

Ontario Health atHome is seeking a Care Coordinator to assess patient care needs, determine eligibility for services, and provide access and referrals to community resources. You will engage with patients, caregivers and health care professionals in a collaborative, patient-centered approach across office, community, or facility settings.

The role emphasizes care coordination, multidisciplinary teamwork, and a commitment to equity, inclusion and patient wellness within Ontario.

Qualifications

  • Membership, in good standing, with the applicable regulatory body: OC Nurses of Ontario; College of Physiotherapists of Ontario; College of Occupational Therapists of Ontario; Ontario College of Social Workers and Social Service Workers.
  • Recent experience in community health or a related field.
  • Knowledge of the health care delivery system and community resources.
  • Excellent interpersonal, communication, assessment/problem-solving, and decision-making skills.
  • Effective time management, prioritization and organizational skills, with ability to work independently and in a busy multidisciplinary environment.
  • Ability to accurately complete required documentation, reports and forms.
  • A valid driver’s license and access to a reliable vehicle.
  • Proficient in a Windows environment.
  • Advanced oral and written proficiency in English and French is essential.
  • COVID-19 vaccination status disclosure is required prior to start date.

Responsibilities

As a Care Coordinator (Full-Time, Start Date: Immediately, Location: Sudbury), you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

What will you do?

  • In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans

  • We have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

Bilingual candidates with advanced oral and written proficiency in English and French will be given first consideration; however, less qualified candidates who don’t meet the language requirements may still be considered.

  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
  • Case management experience or recent related community experience
  • Ability to speak French or another second language
