As a Care Coordinator (Full-Time, Temporary, immediately to December19, 2025; Location: Sault Ste. Marie), you will assess and determinepatient care needs and eligibility, provide access and referrals to communityservices, and engage with patients, caregivers and other health carepractitioners.
Whether you work in our office, in the community, or a health carefacility – you will play a lead role in providing connected, accessible,patient-centered care – and be supported by our collaborative team thatincludes over 9,000 regulated health care and other professionals.

What will you do?
· In collaboration with patients and their families, assess careneeds, determine eligibility for services, and develop individual care andservice plans
· Linkpatients with service providers
· Coordinate and monitor care plan delivery
· Establish a helping relationship with patients and their families
· Balance patient needs and choices with available resources, ensuringpatients’ values and preferences are respected
What must youhave?
· Membership, in good standing, with the applicableregulatory body:
What would give you the edge?
· Experience working with diversepatient groups, e.g., multicultural, homeless, palliative, acquired braininjury (ABI), mental health, geriatrics, pediatrics
· Casemanagement experience or recent related community experience
· Abilityto speak French or another second language
