Basic Function:

The Care Coordinator at FiftyForward works closely with aging adults across the Living at Home, Care Team, and Conservatorship programs. Core responsibilities include conducting in-person client visits, assessments, resource coordination, advocacy, documentation, and supporting departmental operations.

Core Responsibilities:

  • Administer assessments for clients to determine eligibility, needs, and service plans.
  • Develop and update individualized care plans based on client needs.
  • Collaborate with conservatorship staff for accountings, visits, and on-call coverage.
  • Engage with community partners and agencies to support clients holistically.
  • Refer clients to resources and complete applications.
  • Respond to inquiries and connect callers with services.
  • Incorporate evidence-based practices into care decisions.
  • Maintain accurate client records and timely reports.
  • Assist with bill payments and record transactions.
  • Provide transportation as needed.
  • Support internal programs and outreach efforts.
  • Build relationships with community partners and conduct outreach events.
  • Follow NASW Code of Ethics.
  • Perform other duties as assigned.

Core Work Requirements:

Education: Bachelor’s in social work or related field, or three years of equivalent experience.

Preferred Experience: Care management, legal practice, aging services, benefits, or social services.

Additional Skills: Strong interpersonal skills, problem-solving, trauma-informed care, motivational interviewing, cultural competence, knowledge of Middle Tennessee social services, reliable transportation, clean driving record, bilingual in Spanish preferred.

Certifications and Licenses: Valid driver’s license; CPR and First Aid within 90 days of hire.

Physical Aspects: Travel within Davidson County and nearby areas, home visits, potential exposure to odors, animals, clutter, ability to climb stairs, bend, push, pull, stand, and lift up to 25 pounds.

Equipment Requirements: Computer, printer, copier, telephone.

Working Hours: Full-time, 40 hours/week, Monday-Friday 8:00 a.m. to 5:00 p.m., with occasional early mornings, evenings, or weekends.
