Job Summary:

The Care Coordinator is responsible for evaluating client needs, overseeing and monitoring the care delivered, developing appropriate care plans, and ensuring caregiver competency. They participate in establishing and implementing high standards of care.

The Care Coordinator provides leadership in all aspects of client care, including client assessments, health education/counseling, and follow-up care, while upholding Qualicare brand standards.

Responsibilities:

• Consult with supervisors and staff regarding clients, families, and service programs
• Ensure compliance with standards of care
• Conduct in-home assessments and create care plans
• Coordinate with physicians and other care providers
• Perform supervisory visits to clients regularly
• Participate in employee selection and supervise staff performance
• Contribute to quality assurance and risk management programs

Qualifications:

• Current certificate of competence from CRNA or CLPNA, in good standing
• Strong clinical skills
• Self-directed with minimal supervision
• Excellent customer service skills
• Experience in employee supervision
