Job description

• Travel to and assist persons in their private home or community settings.
• Implement person-centered plans designed to support and achieve personal goals (including personal home living skills).
• Instruct, guide, assist, and model socially appropriate skills and behaviors.
• Be aware of the individual’s physical, social, and recreational environments, taking action to minimize risks and maximize participation/inclusion.
• Assist individuals in participating in a meaningful way in their personal life planning.
• Administer medical administration or first aid, according to prescribed procedures.
• Participate in shift exchanges, staff communication, and communication of issues or concerns.
• Ensure comprehensive and accurate recording and reporting of information (e.g., daily logs, incident reports).
