The Behaviour Support RPN reports to the Director of Care and provides clinical support in identifying and managing responsive behaviours. This dedicated nurse will serve in a specialty role, delivering care to older adults with or at risk for responsive behaviours associated with dementia, mental health issues, substance use, or neurological conditions.
The Behaviour Support RPN collaborates with the Behaviour Support Resource Team, Psychogeriatric Resource Consultants, Geriatric Mental Health Outreach Team, Long Term Care Behaviour Support Outreach Team, hospitals, Ontario Health atHome, primary care providers, and other professionals.
Responsibilities
- Provide clinical expertise and comprehensive, client/family-centered health care services.
- Collaborate with Psychogeriatric Resource Consultants and mobile behaviour support resources to develop and refresh the LTCH’s Behaviour Support Resource Team (BSRT).
- Provide leadership and support to the LTC home’s BSRT.
- Facilitate Quality initiative meetings to ensure appropriate medication use, lead resident medication reduction efforts, and implement non-pharmacological strategies to manage expressive behaviours.
- Participate in interdisciplinary care conferences with residents, families, and healthcare team members for the management of residents’ expressions.
- Coach LTC staff in frameworks such as PIECES and UFIRST to assess behaviours, identify triggers, and develop care plans.
- Conduct comprehensive geriatric assessments, treatment, follow-up, and discharge planning in collaboration with the BSRT.
- Guide care plan implementation and transitions.
- Assist in transitioning residents between community, hospital, and other LTC facilities.
- Chair Behaviour Rounds to discuss care plans with staff and external resources.
- Identify and coordinate external referrals and consultations.
- Advocate for residents’ rights and address systemic challenges.
- Identify training needs and support capacity building within the home.
- Participate in onboarding of new staff and volunteers to the Responsive Behaviour program.
- Perform other duties as assigned.
Qualifications
- Current registration with the College of Nurses of Ontario as an RPN.
- CPR/First Aid certification.
- Training in GPA, UFIRST, P.I.E.C.E.S., Dementiability is an asset.
- At least one year of experience in long-term care or with residents in a similar setting.
- Knowledge of responsive behaviours related to dementia and neurological conditions.
- Ability to coach and mentor others.
- Accountability in leading and organizing care activities.
- Experience with quality improvement processes (asset).
- Basic computer skills, including Microsoft Office and electronic communication tools.
- Knowledge of RAI-MDS process.
- Flexible schedule to support all shifts.
- Ability to interact empathetically and turn tasks into positive social interactions.
- Ability to meet legal requirements, promote residents’ rights, and manage risks.
- Complete a current Vulnerable Sector Check and TB testing results.
- Provide proof of required vaccines; staying up-to-date is recommended.
- Two supervisory references.
