The Behaviour Support RPN reports to the Director of Care and serves as an in-house support for identifying and managing responsive behaviours. This dedicated nurse will provide clinical care to older adults with or at risk for responsive behaviours associated with dementia, complex mental health issues, substance use, and/or neurological conditions.

The Behaviour Support RPN will collaborate with the Behaviour Support Resource Team, Psychogeriatric Resource Consultants, Geriatric Mental Health Outreach Team, Long Term Care Behaviour Support Outreach Team, hospitals, Ontario Health at Home, primary care providers, and other professionals.

Responsibilities
  • Provide clinical expertise and comprehensive, client/family-centered health care services.
  • Collaborate with Psychogeriatric Resource consultants and mobile behaviour support resources to develop and update the LTCH’s Behaviour Support Resource Team (BSRT).
  • Provide leadership and support to the LTC home’s BSRT.
  • Facilitate Quality initiative meetings to ensure appropriate medication use, lead efforts to reduce antipsychotic medications, and implement non-pharmacological strategies to manage expressive behaviours.
  • Participate in interdisciplinary care conferences for residents and families to identify and manage personal expressions.
  • Coach LTC staff in frameworks such as PIECES and UFIRST to assess behaviours, identify triggers, and develop care plans.
  • Conduct comprehensive geriatric assessments, treatment, follow-up, and discharge planning in collaboration with the BSRT.
  • Guide care plan implementation and transitions.
  • Assist in transitioning residents between community, hospital, and other LTC facilities.
  • Lead and chair Behaviour Rounds for staff to discuss care plans.
  • Identify needs for external consultations and coordinate referrals.
  • Advocate for residents’ rights and address systemic issues by raising challenges with supervisors.
  • Identify training needs and coordinate capacity-building support.
  • Participate in onboarding new staff, volunteers, and students regarding the Responsive Behaviour program.
  • Perform other duties as assigned.
Qualifications
  • Current registration with the College of Nurses of Ontario as an RPN.
  • Current CPR/First Aid certification.
  • Training in GPA, UFIRST, P.I.E.C.E.S., Dementiability is an asset.
  • Minimum one year experience with residents in long-term care.
  • Knowledge of responsive behaviours related to dementia and neurological conditions.
  • Ability to coach and mentor others.
  • Professional accountability in care leadership and organization.
  • Experience with quality improvement processes is an asset.
  • Proficiency in Microsoft Word, Excel, and online learning tools.
  • Understanding of RAI-MDS process.
  • Flexible schedule to support all shifts.
  • Ability to engage with people’s feelings and turn tasks into positive interactions.
  • Commitment to legal and ethical standards, promoting residents’ rights.
  • Complete a current Vulnerable Sector Check and TB testing results.
  • Provide proof of required vaccines; stay updated with vaccination recommendations.
  • Two supervisory references.
