The Behaviour Support RPN reports to the Director of Care and will serve as an in-house support on the identification and management of responsive behaviours. This dedicated nurse will function in a specialty role providing clinical care to older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.

The Behaviour Support RPN will work in collaboration with the Behaviour Support Resource Team, Psychogeriatric Resource Consultants, Geriatric Mental Health Outreach Team, Long Term Care Behaviour Support Outreach Team, hospitals, Ontario Health at Home, primary care providers, and other providers and professionals.

Responsibilities:
  1. Provide sound clinical knowledge and skills, act as clinical expert and resource, and provide comprehensive, quality client/family centered health care services.
  2. Collaborate with Psychogeriatric Resource consultants and mobile behaviour support resources in the development/refresh of the LTCH’s Behaviour Support Resource Team (BSRT).
  3. Provide leadership and support to the LTC home’s BSRT.
  4. Facilitate Quality initiative meetings to ensure appropriate use of medications in the interest of resident safety and quality of life, lead the process to identify residents for antipsychotic medication reductions and implement non-pharmacological and monitoring strategies to manage expressive behaviours.
  5. Participate in interdisciplinary care conferences with residents/families and health care team members for identification and management of the resident's personal expressions as required.
  6. Coach LTC home staff in the use of frameworks such as PIECES and UFIRST to assess responsive behaviours, identify triggers and develop behaviour care plans and approaches to providing care to residents requiring behavioural support as well as their families.
  7. Provide comprehensive bio-psychosocial geriatric assessments, treatment, follow-up and discharge planning in collaboration with the BSRT.
  8. Provide coaching on the implementation of behaviour care plans and guide appropriate care plan transitions.
  9. Aid in the transition of long-term care residents with responsive behaviours (new or existing) between community, hospital and neighbouring long-term care homes (LTCHs).
  10. Convening and chairing regular Behaviour Rounds that enable front-line staff to discuss care plans with internal and external resources.
  11. Identify the need for additional consultation and/or referrals to external services or specialties and coordinate referrals.
  12. Advocate for and protect the individual rights of residents/clients.
  13. Raise client-specific challenges/barriers to the supervisor, as a means of identifying systemic issues where broader consultations may be required.
  14. Proactively identify training and education needs for the home and coordinate with the PRCs to deliver capacity building support.
  15. Participate in the Orientation process of new employees, volunteers or students in regards to the Responsive Behaviour program.
  16. Other duties as assigned and or indicated in Job Task Inventory.
Qualifications:
  1. Current registration with the College of Nurse of Ontario as an RPN.
  2. Current CPR/First Aid certification.
  3. Training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Dementiability are an asset.
  4. Minimum one (1) year experience with clients in long-term care or residents in a long-term care home setting.
  5. Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.
  6. Demonstrated ability to coach and mentor others.
  7. Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for residents and families.
  8. Experience with quality improvement methods and processes an asset.
  9. Basic computer skills – Microsoft Word, Excel and online learning utilities including ability to use electronic communication devices preferred.
  10. Knowledge and understanding of RAI-MDS process.
  11. Demonstrate flexibility in schedule to ensure all shifts (Days, Evenings, and Nights) get support if needed.
  12. To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction.
  13. To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  14. Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
  15. As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements.
  16. Two supervisory references required.
  17. Provide proof of all required vaccines. The Employer strongly recommends to “stay up to date” with the recommended vaccines. Vaccination requirements are subject to changes as determined by the Employer.
