Job summary

Sienna Senior Living is seeking a Behavioural Support Nurse for Weston Terrace Community in Toronto. This temporary full-time role runs 7am to 3pm and offers a competitive rate per the collective agreement.

You will provide clinical expertise, resident-centered care, and support transitions for residents with responsive behaviours. Ideal candidates are RPNs with LTC experience, trained in GPA/U-FIRST/P.I.E.C.E.S./Montessori/DementiAbility, and legally eligible to work in Canada.

Qualifications

• Registered Practical Nurse in good standing with the Ontario College of Nurses.
• Minimum 3 years’ experience in long term care.
• Knowledge of responsive behaviours in older adults including dementia, mental health, substance use, and other conditions.
• Specialized training in GPA, U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility is an asset.
• Legal eligibility to work in Canada or valid work permit.

Responsibilities

• Provide sound clinical knowledge and act as clinical expert and resource.
• Deliver comprehensive, resident- and family-centered health care services.
• Assist in transitions of residents with responsive behaviours between settings.
• Consult on resident applications and recommend care for responsive/expressive behaviours.
• Screen and assess risk for residents with new or changed behaviours.
• Coordinate referrals and engage community partners in resident care needs.
• Facilitate education sessions to build team capacity.
• Support case reviews and clinical rounds management.
• Engage the interprofessional team in developing individualized care plans.

Reporting to the Director of Care, the Registered Practical Nurse – Behavioural Support Nurse is a critical role within our communities as it impacts the lives of residents.

What you’ll be doing:

You will be a welcomed member of theSienna Senior Living team and impact in the following ways:

• Provide sound clinical knowledge and skills, acting as clinical expert and resource
• Provide comprehensive, quality resident and family-centered health care services
• Aide in the transition of long term care residents with responsive behaviours between community, hospital and neighboring long-term care communities (LTCHs)
• Consult on resident applications and make recommendations to meet the needs related to responsive/expressive behaviours
• Screen and complete risk assessment for residents with new or change in behaviours
• Coordinate referrals and engage community partners in supporting the resident care needs
• Facilitate education sessions to build capacity in team members
• Support the management of case reviews and clinical rounds
• Engage the interprofessional team in the development of individualized plan of care

Who you are

• A Registered Practical Nurse in good standing the Ontario College of Nurses
• Minimum 3 years’ experience working in long term care
• Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions
• Specialized training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility are an asset
• You have good judgement, the ability to identify risk and can foster a positive work environment
• You must be legally eligible to work in Canada or have a valid work permit
