Responsibilities

  • Conduct behavioural assessments per best practice guidelines
  • Identify triggers with patients, caregivers, and staff
  • Implement, evaluate and document behavioural interventions
  • Develop, monitor and evaluate care plans; review documentation and outcomes
  • Provide education, training, coaching to hospital staff
  • Create referrals to additional BSO team members as needed
  • Support transition of patients from hospital to next setting with community links
  • Coordinate with community services and pre-transition planning
  • Develop individualized transitional care plans shared with families
  • Provide information to patients and caregivers for transition
  • Ensure timely information and referrals to Alzheimer Society Peel and BSO
  • Identify staff education needs and make referrals
  • Encourage independence and manage behaviours
  • Coach staff on managing responsive behaviours
  • Observe patients to identify triggers and recommend strategies
  • Participate in care conferences/huddles
  • Maintain contact with medical professionals
  • Collaborate with community partners
  • Maintain monthly statistics via reporting system
  • Prepare administrative documentation and participate in team meetings

The Behavioural Interventionist will support staff and informal caregivers, providing care to individuals with responsive behaviours that are unmanaged and are deemed as a barrier to being able to provide care. These behaviours may be associated with Alzheimer’s disease and related dementias, mental health, substance use, and other neurological conditions.

Accountabilities:

  • Conduct behavioural assessments as set out in Alzheimer Society Peel best practice guidelines
  • Collaborate with patient, caregivers and hospital staff to identify triggers to behaviours
  • Implement, evaluate and document specific behavioural interventions in consultation with elder lifespecialist
  • Develops, monitors and evaluates the care plan recommendations by reviewing documentation, behavioural tracking and outcome measures to illustrate impact
  • Provides patient specific education, training, modeling and coaching to hospital staff involved in patientcare
  • Creates appropriate referrals to additional BSO team members where needed to support the patient, caregiver and staff
  • Provides support to ensure the successful transition of patients from the hospital to their next transitionlocation through:
    • The connection and collaboration with community services as needed to the patient and caregivers
    • Pre‑transition planning which will include the patient and the caregiver; will include the adoption of a standardized risk assessment to identify the transition risks
    • The development of an individualized transitional care plan which will be shared with families and caregivers to support the transition and mitigate the identified transition risk
    • Provide the necessary information to the patients and caregivers to support the transition
  • Ensures patients and caregivers are provided with timely and appropriate information and referral to the Alzheimer Society Peel and BSO services
  • Identifies staff and caregivers who would benefit from additional education and makes the appropriate referrals
  • Encourages independence
  • Implements the necessary protocols to assist in the management of behaviours, facilitate success in managing behaviours and in providing care
  • Coach staff in understanding and managing responsive behaviours
  • Direct observation of patient during times of reported, responsive behaviours to identify triggers/unmet needs and recommend appropriate strategies to manage the behaviour
  • Participates in care conferences/patient huddles as required
  • Maintains ongoing contact with medical health care professionals
  • Initiates and participates in ongoing collaboration with community partners
  • Maintains accurate and timely monthly statistics through the identified statistical reporting system

  • Participates in administrative team meeting provides input and ideas for improving communication, services, client/caregiver support, as part of special projects, quality improvement
Job qualifications
  • Community college diploma in a health care related field (Bachelor’s degree in a health care related field is preferred)
  • Certificate in gerontology preferred or demonstrated evidence towards completion is preferred
  • Gently persuasive approachcertified
  • One year recent experience working with the seniors and those with cognitive impairment in an acute care setting
  • Experience with the prevention and management of geriatric syndromes such as delirium/BPSD preferred
  • Attention to detail and expectation to meet documentation standards
  • Excellent communication (written and verbal) and interpersonal skills
  • Demonstrated ability to work as part of an inter-professional team
  • Ability to work independently: initiating, prioritizing and managing multiple tasks
  • Will be required to travel between all Osler sites

Additional information

Hours: Currently Days, 8 hour shifts (subject to change in accordance with operational requirements)

Salary:
Minimum: $35.27 per hour
Maximum: $44.10 per hour
