The Opportunity

The Behavioural Support Coordinator supports the Social Worker, Assistant Director of Care, Director of Care, Nurse Managers, residents, and their families. The Coordinator works within the guidelines of the Central East LHIN Behavioural Support Ontario Project to coordinate care and services for residents with responsive behaviours.

What Is In It For You
  1. Utilize effective communication and interpersonal skills with residents, family members, and care partners.
  2. Ensure current care plans are in place for residents with responsive behaviours, guiding staff and mitigating risks.
  3. Liaise with external partners to support resident-centered care.
  4. Conduct resident case reviews to support care plans.
  5. Address resident or family concerns appropriately, escalating when necessary.
  6. Participate in quality improvement activities and report monthly statistics on responsive behaviours.
  7. Attend the Professional Advisory Committee quarterly.
  8. Collaborate with pharmacists and physicians, understanding medication impacts on resident care.
  9. Demonstrate understanding and commitment to the residence's philosophy, policies, and the Behavioural Support Ontario Project.
What You Will Need
  1. Knowledge of dementia, delirium, mental health issues, and their impact on residents and families.
  2. Maintain RPN certification and good standing with the College of Nurses of Ontario.
  3. Possess and maintain specialized qualifications relevant to this position.
