Awake Overnight Program Services Case Worker

Job Responsibilities

• Maintain clear written and verbal communication with clients and team members.
• Complete all documentation related to client intake/discharge, case notes, incident reports, medication records, and statistical reports.
• Manage and update personal and work-related accounts (email, scheduling, corporate systems, Microsoft Teams, etc.).
• Stay informed on all communications, including emails, meeting minutes, policy updates, and training materials.
• Report monthly performance statistics and incident reports, including internal and critical incident reports.
• Maintain daily responsibility logs and bed charts.
• Participate in ongoing performance appraisals and coaching sessions.
• Provide case work focused on housing stability and support client housing-related needs using HIFIS system.
• Coordinate with professional and agency support staff.
• Assist with moderate cleaning and facility upkeep when necessary.
• Adhere to all responsibilities outlined on shift responsibility sheets.

Qualifications

• College diploma or equivalent in social services; Bachelor of Social Work preferred.
• Experience with homelessness, mental health, and addictions.
• Knowledge of women’s issues related to homelessness and mental health.
• Commitment to anti-racism, anti-oppression, and equity principles.
• Strong confidentiality, communication, and computer skills.
• Ability to work independently and as part of a team.
• Good interpersonal and time management skills.
• Knowledge of community resources and strong empathy for clients, including difficult-to-serve youth.
• Ability to provide a Vulnerable Sector Screen.

Compensation
