Responsibilities
  • In this role as an Attending Physician, you will work collaboratively with the interdisciplinary team, including other APs, NP, RNs, RPNs, PSWs, to provide medical care for residents.
  • In collaboration with the Medical Director, provides comprehensive primary care to residents.
  • Collaborates and consults with the Director of Care, Nurse Practitioner, nursing staff, interdisciplinary team and external resources regarding residents' plan of care.
  • Attends the home on a weekly basis to provide clinical care to assigned residents.
  • Addresses resident acute and chronic care needs and provides appropriate and timely documentation.
  • Identifies circumstances in which palliative and / or end-of-life care may benefit the resident and family members and work collaboratively with members of the interdisciplinary team.
  • Participates in after-hour on-call schedule
  • Conducts regular medication drug review and documents / updates residents' health records.
  • Participates in outbreak management meetings and provides on-site medical oversight during outbreaks
  • Commits to continuing education related to long term care, care of the elderly, palliative care and dementia care.
  • Actively participates as a member of the Professional Advisory Committee.
  • Participates in in-service education programs each year.
Qualifications
  • Medical practitioner licence to practise medicine in the Province of Ontario, and always be a member in good standing of the College of Physicians and Surgeons of Ontario.
  • Experience working in long-term care, preferred.
  • Knowledge of regulations governing long-term care.
  • Ability to work effectively as part of an interdisciplinary team
  • Ability to build trust and interact in a professional manner with staff, residents, families, and other community partners.
