We are looking for someone to lead the development of a healthcare system for our North Toronto community.
Sunnybrook Health Sciences Centre, in Toronto, Canada, a fully affiliated academic Health Sciences facility of the University of Toronto, is inviting applications for its new Chief, Integrated Community Program (ICP). This is an exciting opportunity for an individual who is motivated by change, sets visionary goals, and achieves success by partnering with others. The Program Chief, Integrated Community Program will help to create the medical model for truly integrated care which supports people remaining healthy in our community, and allows for seamless access to specialized services when and where they are needed.
Sunnybrook has recently undergone an integration with Vibrant Community Health, creating a unique model where a hospital and a community health centre are jointly creating a health hub for North Toronto that will advance attachment to interprofessional, academic family medicine for our community. The Cipriano Centre for Seniors Health, which will open in the fall of 2025, will serve as a hub for the delivery of specialized geriatric care both at the hospital and in our community.
Sunnybrook’s Integrated Community Program encompasses a range of inpatient and outpatient services, with internal accountability to support strategic visioning for the program. The Integrated Community program includes Family and Community Medicine, Specialized Geriatric Services, General Internal Medicine and several medical subspecialties, Acute Care Surgery, Dentistry and Nephrology and Dialysis. It also provides oversight for post-acute care, whether in settings such as our Pine Villa or our Reactivation Care Centre transitional care units, or at home through our Sunybrook-2-Home program. By breaking down silos between hospital and home, we will provide ‘wrap around’ care for patients to ensure consistency in quality and access to services across the system of care.
What will success in this role look like?
Sunnybrook’s vision is to invent the future of healthcare. This includes building a health system around the needs of more than 1.1 million patients who come through our doors each year, as well as extending our reach deep into our community. In this role, the successful candidate will build partnerships with care providers and agencies in the community we serve to support a truly integrated system for the delivery of care, which is less hospital-centered. The Chief will provide institutional leadership on issues related to the Integrated Community Program and, in partnership with the clinical, operational and executive leadership, help to create and support effective and efficient models of care.
Qualifications
The successful candidate may be a physician or a regulated health professional. Candidates must be eligible to hold a clinical full time academic appointment at the rank of Associate Professor or Full Professor at the University of Toronto.
