THE ROLE IN A NUTSHELL

The Assessment & Treatment Case Manager (ATCM) will be responsible for improving the customer experience with Substance Use / Work Health Assessments and Post Assessment Treatment Services. The ATCM will assist and monitor clients’ compliance with, and fulfillment of, coordinating and attending assessment appointments as well as fulfilling recommendations outlined in the assessment report. This position also provides administrative support to Clinical Managers and other key stakeholders.

  • Facilitate setup of Clinical Services, including bridge counselling, assessments, and post-assessment services
  • Facilitate referrals to inpatient and outpatient treatment
  • Monitor counselling attendance & progress toward completing assessment recommendations. Provide updates on a monthly, bi-weekly, or weekly basis depending on client needs
  • Gather session updates, track canceled/no-show appointments
  • Prepare reports for Clinical Managers’ review
  • Provide regular updates to the customer contact, including case discussions and written case status documentation
  • Collaborate closely with Clinical Managers to ensure efficient case management and communication of updates
  • Update summary documents upon meeting completion

WHO WE'RE LOOKING FOR

  • Bachelor's Degree in Social Work, Social Sciences, or Psychology
  • 3-5 years experience in case oversight and/or training in mental health; experience or training in substance use disorders is an asset
