Job Overview:

In this position, the APP performs an expanded nursing role by providing primary care to amyloid patients, including taking histories, assessments, ordering labs, and reporting diagnostic lab values to physicians in accordance with Joint Commission, CMS, and RWJUH guidelines, regulations, and policies.

  • Coordinates clinical and administrative processes for patients through all phases of care management
  • Educates patients, families, and staff regarding cardiac disease
  • Collaborates with physicians and staff to evaluate and manage patients
  • Completes data analysis, reporting, and other administrative functions for the cardiac amyloid program
  • Participates in the care of both inpatients and outpatients
Qualifications:
  • Current Licensure as an Advanced Practice Nurse or Physician Assistant in New Jersey
  • NCCPA Certification required
  • ACLS & BLS Certifications
  • DEA & CDS registration
  • Experience in managing cardiac amyloidosis and other cardiomyopathies is required
Scheduling Requirements:

Essential Functions:
  • Practice within inpatient and outpatient adult populations under RWJUH Cardiovascular Service
  • Develop and review collaborative practice agreements specific to clinical areas
  • Perform assessments, including history and physical examinations, prior to prescribing medications
  • Manage assessments and diagnoses following accepted advanced nursing standards
  • Evaluate cardiac amyloid, sarcoid, hypertrophic cardiomyopathy, and other patients
  • Prepare cases for multidisciplinary meetings
  • Perform therapeutic and diagnostic procedures, consultations, and lab examinations
  • Coordinate patient evaluations and insurance clearances
  • Communicate with referring physicians and healthcare providers
  • Engage with families regarding patient's medical care and progress
  • Reinforce education, follow-up, and medication adherence
  • Ensure follow-up testing and data reporting
  • Arrange patient admissions, transfers, and discharges
  • Document medical histories and physical exams
  • Conduct daily rounds, including clinical tasks and documentation
  • Coordinate referrals and specialist consultations
  • Write discharge summaries and coordinate care transitions
  • Prescribe or recommend treatments and medications
  • Maintain knowledge of inpatient care plans and respond to patient needs
  • Participate in departmental research and quality metrics review
