Job description

Adult Protective Support Worker/Case Manager

POSITION SUMMARY:
FSTV is currently accepting applications for a full-time Adult Protective Service Worker (APSW)/Case Manager position. The position reports to the Director of Community Integration Program. The APSW coordinates services for people with a primary diagnosis of developmental disability and may also have complex needs including mental health/addictions, criminal justice, housing access, and medical situations. The APSW manages a caseload of individuals, helping them become self-determining, secure, and fully integrated into the community through natural supports and community resources. The APSW also advocates for community connection and integration for people with developmental disabilities.

REQUIRED QUALIFICATIONS, EXPERIENCE, AND SKILLS:

  1. A human services diploma and/or bachelor’s degree in a related field is mandatory
  2. Minimum of 5 years of experience supporting individuals in outreach to community supports and a commitment to increasing knowledge and skills as a support worker
  3. A strong and demonstrated belief in human rights, respect for all citizens, and belief in the right of people to make their own choices
  4. Understanding and background in developmental services with extensive knowledge of community resources
  5. Experience providing education, coaching, or mentoring as part of a community-based program
  6. Strong understanding of the social determinants of health affecting people’s health, well-being, and quality of life
  7. Ability to cultivate positive and collaborative relations within the community
  8. Demonstrated experience functioning as an advocate
  9. Strong interpersonal skills with the ability to establish trust and rapport with individuals of diverse backgrounds, capacities, and needs
  10. Ability to manage oneself, handle stressful situations, meet deadlines, adapt to changing priorities, and conduct oneself professionally
  11. Ability to work in a consultative and collaborative manner with respect and dignity for others
  12. CPR/First Aid certification required
  13. Access to a vehicle and a current driver’s license
  14. Clear current Police Vulnerable Sector Check (PVSC)
