Job summary

Stonehenge is seeking an Addiction Support Coordinator to provide direct services for individuals with addiction and co-occurring conditions. Most work occurs in clients’ homes; travel across the Waterloo Wellington LHIN is required, including rural areas.

Occasional evenings and weekends may be required. Responsibilities include counselling, coordinating access to primary care and supports, risk assessment, referrals, and discharge planning, with collaboration across community agencies.

Qualifications

• BSW/MSW or other Masters’ level counselling qualification required
• 3 - 5 years of experience in addictions, mental health and case management
• Preference for registration with a professional college
• Valid First Aid/CPR (Level C)
• Valid G Driver’s License and satisfactory Motor Vehicle Record (G2 level or greater)
• Reliable vehicle and willingness to transport clients with proof of $2M liability coverage

Responsibilities

• Counselling services and 1:1 interventions to support mental health and addiction goals
• Coordinate intake and referrals to primary care, addiction, and mental health supports
• Assess risk and safety with crisis planning and intervention
• Maintain detailed records and complete reports
• Discharge planning and collaboration with community agencies
