Interested applicants who need a PVSC are encouraged to apply for one promptly to avoid delays.WHAT WE'RE OFFERING:- Starting rate for this role is $27.14 per hour (Level 1 – Direct Service), with a wage differential based on regulation ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Candidates registered with designated professional colleges (e.g., OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation.
- External applicants start at Level 1, with progression up to Level 8 ($34.41 non-regulated, $37.26 regulated).
- Internal applicants' grid level placement will follow the Collective Agreement.
Work schedule: 70 hours over two weeks, 9 am - 4:30 pm Monday-Friday or Compressed Work Week
Benefits: 9% in lieu of benefits
Pension: Eligible for Healthcare of Ontario Pension Plan enrollment
HOW YOU WILL MAKE A DIFFERENCE:Operations:- Support implementation of new initiatives and policy changes;
- Coordinate with Team Lead and Manager on discharges and admissions;
- Assist with intake assessments to determine program fit;
- Problem solve and provide feedback on program concerns;
- Ensure program consistency and respectful, accessible service delivery.
Primary Worker:- Designated primary worker for approximately 20 tenants;
- Maintain complete and current clinical records;
- Support tenants in maintaining housing and meeting their needs.
Direct Service:- Support tenants with tenancy tasks, life skills, social support, health and wellness, personal support, community linkages, crisis intervention, eviction support, motivation, and health education.
Clinical Assessment, Support, and Goal Planning:- Assess individual strengths, needs, and preferences; develop wellness plans; involve natural supports;
- Evaluate social/environmental factors and risk;
- Gather feedback on progress and adjust plans accordingly.
Communication and Decision Making:- Maintain open communication with supports and staff;
- Make collaborative decisions;
- Complete discharge summaries and documentation;
- Liaise with community agencies;
- Manage emergency situations and ensure confidentiality.
WHAT YOU'LL NEED TO APPLY:Police Vulnerable Sector Check: Must be recent (within six months).
Qualifications:- College diploma in social services or related field; original credentials required.
- Valid driver’s license and vehicle access.
- Clear PVSC.
- Reliable attendance.
Preferred Qualifications:- Bachelor’s degree in social work, psychology, or nursing;
- Registration with relevant college;
- Problem-solving, initiative, team, and leadership skills;
- Effective communication and time management;
- Knowledge of mental health, medications, community resources, documentation, and safety;
- Experience with mental health or addictions;
- Trauma-informed care training;
- Housing experience and knowledge of the Residential Tenancies Act;
- Certifications in NVCI, CPR/First Aid, ASIST;
- Bilingual in both official languages preferred.
PHYSICAL REQUIREMENTS:- Standing and walking for extended periods;
- Ability to climb stairs, as homes are not fully accessible.
