Participates as a member of the interdisciplinary team to plan, implement and evaluate programs, outings and events that meet the residents’ physical, social, emotional, cognitive, spiritual and cultural needs.

Responsibilities
  • Complete initial and ongoing programs assessments as necessary to determine residents’ recreation and rehabilitation needs.
  • Complete documentation and accurate records as required including: Program guides, participation tracking, monthly statistics, Intake documentation, Quarterly documentation, MDS / RAPS, care plan updates and action plans.
  • Plan and implement programs based on resident assessments, offering new, creative and innovative recreational programs to residents on an ongoing basis
  • Facilitate one-on-one and small group programs on home unit
  • Support and roll out self-directed programs
  • Show evidence of being focused on people’s feelings when providing activities and knowing how to turn a task into positive social interaction
  • Plan and implement special events, community-based programs and outings
  • Evaluate and make adaptations to programs as appropriate
  • Complete equipment assessments and ensure that resident has access to appropriate assistive devices as required (i.e. wheelchair, walker)
  • Provide most appropriate available wheelchair / walker for new admissions and current residents in response to a change in status
  • Track and label Rehabilitation / Recreational equipment on unit
  • Develops programs in Welbi.
  • Provide residents with a calendar of programs offered by Welbi
  • Ensures that posters, notices, and calendars are up to date, attractively displayed and easy for residents, families and team members to read.
  • Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
  • Other duties as assigned
Qualifications
  • Has a post-secondary diploma or degree or certificate in recreation and leisure studies, therapeutic recreation, gerontology or other related fields from a community college or university
  • Experience working with seniors
  • Knowledge of resident population and programming for dementia and mental health.
  • Experience planning, implementing and evaluating programs
  • Ability to use your imagination to engage others
  • Computer literate
  • Excellent written and verbal communication skills
  • Excellent organization skills
  • Second Language (Chinese, Portuguese, Italian, and Eastern European languages are an asset)
