Job summary

WoodGreen Community Services seeks a dedicated activator to plan, develop, and evaluate programs for seniors and caregivers. The role is onsite at 1080 Queen Street East, with a schedule of two days (Sat 9:00–17:00, Sun 9:00–17:00) totaling 14 hours weekly.

The successful candidate will lead groups, co-design individualized plans, ensure quality service, and support clients and caregivers while coordinating with a multidisciplinary team. Fluency in English is essential.

Qualifications

• Two-year college diploma in activation or gerontology.
• Two years of demonstrated experience designing and leading activities for frail and/or cognitively impaired adults.
• Proficiency in English (spoken and written).

Responsibilities

• Plan, develop, implement, and evaluate programs and activities for clients.
• Lead groups and manage daily operations of the program.
• Monitor and adjust activities to client needs and abilities.
• Co-design and facilitate individualized program plans with clients and caregivers.
• Ensure quality service standards per guidelines.
• Encourage client participation in activities addressing physical and psycho-social needs.
• Provide support and education to clients and caregivers.
• Recognize and manage responsive behaviours and escalate as needed.
• Contribute to community outreach activities of the program.
• Monitor clients’ personal, safety, and hygiene needs.
• Handle billing, data entry, documentation, statistics, meal orders and transportation arrangements.
• Escort and/or transport clients to and from the program, ensuring safe paths.

Program Overview – Seniors Community Services, Community Care

The Community Care Unit provides a holistic approach to care at home for seniors and caregivers who are vulnerable, at risk and/or marginalized. The unit provides a wide range of programs such as Assisted Living Services, Social Work, Meals-on-Wheels, Adult Day programs, and transportation, which enable seniors to be as independent as possible in their homes. The unit also provides a variety of socio-recreational/wellness programs, which enhance quality of life and promote the health of older adults.

What You Will Do

• Plan, develop, implement, and evaluate programs and activities that encourage, maintain, and promote dignity and self-worth in clients.
• Lead groups through a schedule of activities and maintain daily operations of the program.
• Monitor programs to ensure that activities are being delivered according to the program goals and make adjustments according to needs, abilities, and interests of the clients.
• Co-design and facilitate individualized program plans with clients and caregivers.
• Ensure quality service standards are met in accordance with operational guidelines.
• Motivate and encourage clients to participate in individual and group programs that address their physical and psycho-social needs as well as Activity of Daily Living Skills.
• Provide support, information and education to clients and their primary caregivers.
• Recognize and manage responsive behaviours and report escalations to Supervisor working with various professional and resource persons to de-escalate issues.
• Contribute to the community outreach activities of the program.
• Monitor and attend to clients’ personal, safety, and hygiene needs.
• Conduct billing, data entry, documentation, statistics, meal orders and transportation arrangement for clients.
• Escort and/or transport clients to and from the program and take steps to ensure pathways are clear from snow, etc.

What You Bring to the Team

• 2 years college diploma in activation or gerontology.
• 2 years of demonstrated experience designing and leading activities for frail and/or cognitively impaired adults.

• Excellent communication skills in working with seniors.
• Good knowledge and skills in leading group activities and providing personal care services.
• Good knowledge and skills in behavioural management techniques.
• Comfortable working with staff and clients from diverse background.
• Strong computer skills in Windows environment.
• Second language(s) to serve the client group(s) considered a strong asset.
• Training in CPR and First Aid is an asset.
