Position Summary

Queer Yukon is seeking a Health Navigator Assistant to support 2SLGBTQIA+ people in accessing and navigating health and social services. This role is based largely out of our Whitehorse office, where the Navigator will be available during drop-in hours to meet community members directly and help connect them with the care they need. The work is practical, people-centered, and often rooted in everyday problem-solving, whether that’s helping someone book an appointment, understand their options, or feel supported in pursuing gender-affirming care. It’s a role that combines advocacy, relationship-building, and hands-on support in a community setting.

Key Responsibilities
  • Community Engagement & Outreach
  • Build and maintain strong, respectful relationships with 2SLGBTQIA+ individuals, community members, leaders, and organizations.
  • Plan and implement outreach activities, events, and workshops that promote awareness, visibility, and inclusivity.
  • Engage with the community through in-person and digital channels (e.g., social media, events, forums).
  • Advocate for equitable treatment, inclusive policy, and the well-being of 2SLGBTQIA+ individuals.
  • Education & Empowerment
  • Deliver culturally competent educational workshops on topics such as HIV prevention (including PrEP), STI prevention, and gender-affirming care.
  • Support clients in understanding health information, lab results, and treatment options.
  • Provide crisis response and peer-based emotional support.
  • Navigation & Case Support
  • Assist clients in developing care plans and accessing healthcare, social services, legal support, and mental health resources.
  • Coordinate appointments, referrals, and follow-ups, including with the Clinical Supports and updating referral lists regularly.
  • Advocate for clients within the healthcare system to ensure they receive respectful and affirming care.
  • Collaboration & Community Partnership
  • Collaborate with healthcare providers, social services, and community organizations to enhance access and service quality.
  • Identify and cultivate opportunities for community building, support networks, and partnerships that benefit 2SLGBTQIA+ Yukoners.
  • Systems & Reporting
  • Maintain accurate and confidential client records.
  • Complete timely and accurate reporting for internal tracking and external funders.
  • Stay up to date on local, territorial, and national resources and services relevant to the 2SLGBTQIA+ population.
  • Social Determinants of Health & Harm Reduction
  • Address systemic and social barriers such as housing instability, transportation, and food insecurity.
  • Promote harm reduction strategies related to sexual health and substance use and education.
Qualifications

Education & Experience

  • Bachelor's degree in social work, psychology, counseling, public health, or a related field preferred but not required.
  • In lieu of a degree, a minimum of 2 years of experience working directly with 2SLGBTQIA+ individuals is required.
  • Experience working with individuals at high risk for HIV/STIs or those seeking gender-affirming care is highly valued.

Skills & Knowledge

  • Strong interpersonal, written, and verbal communication skills.
  • Ability to work independently and collaboratively in a team setting.
  • Excellent organizational and time management skills, with the ability to meet deadlines.
  • High level of cultural competency regarding 2SLGBTQIA+ issues and identities.
  • Understanding of social determinants of health and the specific barriers faced by LGBTQIA+ individuals in accessing care.
  • Experience with documentation, data collection, and client tracking systems.
  • Ability to work flexible hours, including evenings and weekends as needed.
Additional Assets
  • Lived experience as a member of the 2SLGBTQIA+ community.
  • Experience working with or within Northern or First Nations communities.
  • Multilingual abilities are considered an asset.

If this position feels like a good fit for your skills and experience, we encourage you to apply. You don’t need to meet every single qualification to be considered—we value lived experience and a willingness to learn. This is an opportunity to contribute to meaningful, on-the-ground support for 2SLGBTQIA+ Yukoners while being part of a team committed to making services more accessible and inclusive.
