Under the direction of the Manager, Cardiovascular Rehabilitation and Cardiac Diagnostics, the Kinesiologist is responsible for the planning, coordination and supervision of the daily activities of the exercise, education and counseling programs within the Cardiovascular Rehabilitation Program. The Kinesiologist acts as the program participant’s case manager, who develops care plans and actions to achieve increased knowledge and skills to reduce modifiable risk factors, improving participant’s quality of life and reducing risk for future events. The Kinesiologist will work within the interprofessional team to develop the plan of care for individuals attending the Cardiovascular Rehabilitation Program. In addition, the Kinesiologist will participate in the provision of all aspects of the program, including regionally based activities.

EMPLOYMENT REQUIREMENTS:

Education/Experience:

  1. Bachelor of Science degree in Kinesiology
  2. Unencumbered registration with College of Kinesiologists of Ontario
  3. 1‑3 years experience working in chronic disease management; specifically building and progression of care plans

Skills/Abilities:

  1. Minimum 1‑3 years of recent experience working chronic disease management setting
  2. Must demonstrate excellent interpersonal communication skills (electronic, oral and written)
  3. Demonstrated ability to work collaboratively in an interprofessional team environment
  4. Knowledge and ability to adopt and apply policy and procedures
  5. Ability to clinically appraise practice related to quality improvement, ethical and clinical issues
  6. Ability to self‑direct activity to meet target timelines
  7. Demonstrated regular attendance.
  8. Demonstrated computer literacy with Microsoft Office applications including Word, Power Point, Excel, and Access
  9. Demonstrated cultural sensitivity and awareness.
  10. Excellent problem solving, decision making, and critical thinking skills
  11. Excellent organizational and time management skills
  12. Experience in physical assessments
  13. Experience building and presenting patient and family education material

Assets:

  1. Successful completion of Kinesiology Graduate Diploma is an asset
  2. Successful completion of ACSM Exercise Specialist program

Salary Range: $39.160 to 50.770
