Job description

Job Title: Patient Flow Coordinator

Under the direction of the Manager ‑ Patient Flow, the Patient Flow Coordinator is responsible for planning and coordinating the day‑to‑day activities related to patient flow and interprofessional teamwork within the defined patient care unit or program. The Coordinator manages the overall processing of new patient referrals and internal consultations, as well as facilitating the efficient flow of patients through various stages of hospitalization.

The Coordinator works within the program leadership team, which includes the program director, physician leader, unit manager, clinical nurse specialist, nurse practice leader, and interprofessional team members, providing leadership to nursing and interprofessional team members.

Qualifications:
  • Current unrestricted registration as a Registered Nurse with the College of Nurses of Ontario, or the ability to become licensed with the College of Nurses of Ontario
  • Current unencumbered registration with the College of Nurses
  • Minimum 10 years of clinical expertise with the patient population
  • Demonstrated leadership experience and collaborative leadership style
  • Discharge planning experience, specifically in coordination and delivery of services, with knowledge of patient flow concepts
  • Effective communication skills, including providing constructive feedback and resolving conflicts
  • Ability to build relationships and work within a team
  • Knowledge of team-building principles and interprofessional collaboration
  • Knowledge of professional practice standards and guiding principles to foster professional practice and accountability
  • Knowledge of community agencies, long-term care, and other healthcare resources within the community and region
  • Adherence to PFCC philosophy and knowledge of CNO standards of practice
  • Knowledge and expertise of patient flow
  • Ability to work under pressure and manage competing work demands
Additional Information:

Salary Range: $43,070 to $60,000
