Qualifications

Job Title: RPN - RAI Coordinator- LTC

Status: Temporary Full Time (6 Month Contract)

Location: Lakeridge Health Long Term Care AP

Wage Schedule: $36.140 - $38.160 per hour

This position is open to all qualified CUPE staff throughout Lakeridge Health commencing 31 July 2025 and closing at 11:59 pm on 8 August 2025.

Position Summary

Reporting to the Director of Operations, the RAI Coordinator will be accountable for the day to day operations of the RAI-MDS assessment tool at the Lakeridge Gardens 320 bed long term care home built on the Ajax-Pickering hospital site.

Having opened to residents in early 2022, this new home is part of the government's plan to improve long-term care delivery and capacity in Ontario and in the Durham Region. This state-of-the-art long-term care home sets a new bar for seniors' care, with dining and outdoor space on every floor, a large courtyard, onsite diabetes care and other important innovative features.

Just as Lakeridge Health is a beacon in the Durham community, the new long-term care home will be integral to managing and delivering on the needs and expectations of seniors in the Durham community and Ontario.

The RAI Coordinator Will

  • Coordinate the RAI-MDS process within the home in accordance with MOHLTC legislation, LHIN agreements, College Standards of Practice and CIHI requirements
  • Prepare and maintain a schedule of RAI assessments for all disciplines and monitor regularly to ensure timely completion
  • Collaborate with registered staff in identifying any significant health status changes which may warrant additional MDS assessments;
  • Provide instruction and education to front line staff and management regarding RAI MDS, RAPS and care planning;
  • Act as subject matter experts within the homes in regards to clinical coding of the RAI instrument
  • Coordinate and ensure completion of the annual AIS competency evaluations for all staff
  • Compile and submit the quarterly electronic data submission to CIHI
  • Review, analyze and trend quality indicators and share results and findings with management team
  • Ensure compliance with PHIPA and obtain and record consent for Integrated Assessment Record
  • Run and analyze the Rug Weighted Patient Day Report quarterly and report current CMI financial funding levels to the Assistant Administrator
  • Analyze data reports and ensure all residents are classified in the appropriate funding levels, and adjust records as necessary to account for changes or increases in acuity, specialized medical services or clinically complex interventions

Qualifications

  • A diploma in Practical Nursing and a current General Certificate of Registration as a RPN with the Ontario College of Nurses
  • Previous RAI/MDS certificate training, is required.
  • Completion of Master Level 6 AIS training, or willingness to complete within the first 6 months of employment, is required.
  • A minimum of one year previous experience in a RAI/MDS position in a Long Term Care facility
  • Sound knowledge of Canadian Institute of Health Information (CIHI) requirements, Local Health Integrated Network (LHIN) Accountability Agreements, Long Term Care Homes Act and Regulations
  • Knowledge of Ontario College of Nurses Standards of Practice, MOHLTC funding, quality indicators, RAI outcomes and RUG Groupings
  • Familiarity of Personal Health Information and Privacy Act (PHIPA), Electronic Health Record Management and Resident Quality Inspection protocols
  • Strong clinical assessment skills, coupled with good judgment and problem-solving skills
  • Demonstrated computer proficiency, along with excellent knowledge of clinical software systems Well-developed organization and time management abilities
  • Demonstrated ability to lead, coach and mentor others
  • Must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months;
