The Durham Ontario Health Team is looking for an energetic self-starter to support planning and coordination of the Durham Ontario Health Team at Home Program. The Operations Coordinator will assist the Director with key programs and projects within the at Home program. Responsibilities include ensuring high-quality, safe, and consistent service delivery, supervising team members to ensure patient-centered, timely, and efficient services, and promoting ongoing performance improvement while maximizing resource use for service excellence.


Duties And Responsibilities

Quality Management and Improvement Initiatives

  • Oversee daily operations within the Durham Ontario Health Team at Home Program
  • Support development and implementation of initiatives and projects
  • Assist in planning, implementing, and evaluating patient-focused care and services

Daily Operations

  • Promote a safe work environment and participate in developing safe work practices and innovations
  • Collaborate with the Patient Care Manager at Lakeridge Health, system partners, and service providers to contribute to performance appraisals and management
  • Support the development of working groups, committees, staffing schedules, and act as a stakeholder support contact
  • Work with the Program Director to implement and evaluate operational plans aligned with program goals
  • Ensure effective resource utilization within budget
  • Facilitate team building, problem solving, and conflict resolution
  • Coordinate with internal and regional stakeholders
  • Promote excellence in patient-centered customer service
  • Demonstrate intermediate skills in Microsoft Office and web-based systems
  • Manage electronic databases and develop dashboards and reports

Qualifications
  • Bachelor's degree in health sciences or health administration required
  • Registration with a Regulated Health Care Professional College in Ontario preferred, in good standing
  • 3-5 years of experience in operations or project coordination
  • Proficiency in Microsoft Office
  • Strong interpersonal, communication, and project management skills, including knowledge of LEAN methodology
  • Strong analytical skills (quantitative and qualitative)

  • Understanding of safety programs and practices, including participation in regular in-service training
  • Ability to organize, prioritize, and manage demands in a dynamic environment

  • Excellent presentation, written, verbal, critical thinking, and problem-solving skills
