Job Title: Pharmacist

Status: Temporary Full Time (~10 months)

Department: Pharmacy - Oshawa

Program: Pharmacy Services

8 Hour Shifts - Days, Evenings, Weekends & On-Call (Subject to Change)

Location: Lakeridge Health Oshawa

Wage Schedule: $48.210 - $64.500 per hour

Duties And Responsibilities

The Pharmacist is responsible for meeting the medication related needs of patients which will include resolving and preventing drug related problems. The principle responsibilities include working with patients and the healthcare team to provide direct patient care, participating in patient education, facilitating medication reconciliation activities, teaching of students, providing education to nursing and medical staff, and supporting the drug distribution system.

Qualifications

  • Current registration in good standing with the Ontario College of Pharmacists in Part A of the register;
  • Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values
  • Strong clinical assessment skills in identifying and solving drug related problems;
  • Excellent medication counseling skills;
  • Demonstrated ability to research and provide drug information effectively to patients and other members of the multidisciplinary team;
  • Demonstrated excellent organizational skills;
  • Demonstrated ability to work effectively within an interprofessional team;
  • Demonstrated effective interpersonal and verbal and written communication skills;
  • Proven ability to exercise independent judgement and to assess situations and problems efficiently and effectively;
  • Previous hospital experience desired;
  • Demonstrated commitment to our core values of Compassion, Innovation, Courage and Trust; and
  • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area

Conditions Of Employment

The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.

Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
