Wage Schedule: $73.570 - $77.340 per hour

Reporting to the Director, Emergency Services & CEPCP, the Nurse Practitioner (NP) - GEM will work as a RN in the Extended Class RN (EC). The NP will provide leadership and promote excellence in patient-family centered nursing care through expert clinical practice, education, consultation, and research. Based in the Emergency program, the NP will function with a high level of autonomy and expert skill in the management of ED geriatric patients.
Responsibilities:
- Provide direct patient care, coordination of care, consultation, discharge planning, and the coordination of related community and other resources for the geriatric emergency patient population;
- Conduct in-depth comprehensive geriatric assessments of patients that integrate a wide range of complex information;
- Utilize a comprehensive theoretical knowledge base and advanced level of clinical competence in caring for patients;
- Collect and interpret data about the health of the patient to determine a medical diagnosis and treat complex responses of individuals and families to actual or potential problems;
- Initiate and evaluate selected medical and nursing therapies within the NP scope of practice, medical directives, and practice guidelines;
- Collaborate with physicians when determining an acute or chronic diagnosis as appropriate;
- Mentor and tutor nursing and other health professional students as appropriate and participate in continuing education activities to integrate the role of the NP in the program and practice settings;
- Identify quality initiatives that will strengthen and develop nursing professional practice;
- Initiate, participate, and/or lead programs, presentations, publications, evaluations, research initiatives, and other system changes to enhance interprofessional teams;
- Partner with consultants, nursing, and other members of the healthcare team to provide comprehensive medical assessment, management, and education.
Qualifications:
- RN (EC) registration with the Ontario College of Nurses, with a Nurse Practitioner certificate in Acute Care or Primary Care;
- Masters of Nursing required; with clinical focus on gerontology preferred;
- Member of the RNAO and the Nurse Practitioner's Association of Ontario;
- Two (2) years recent clinical experience in Emergency Services preferred;
- Gerontological nursing practice, an asset;
- Successful completion of College of Nurses approved education to prescribe controlled substances required;
- ENC certification preferred or started within one year of start date of position;
- Canadian Nurses Association certification in gerontological nursing, an asset;
- Demonstrated experience with completing comprehensive geriatric assessments;
- Experience in community nursing;
- Experience in evaluating quality of care and patient outcomes within the specialty population, in partnership with physicians and other members of the inter-professional team;
- Experience with research methodology;
- Ability to perform duties professionally and courteously;
- Ability to produce high-quality work while meeting deadlines;
- Demonstrate initiative and good judgment with the ability to multi-task.
