Summary of Duties: Under the direction of the Nursing Coordinator or designate, responsible for resident care in accordance with Extendicare’s philosophy and policies, including nursing, resident care, and administrative duties in the Nursing Department. Utilizing a primary model of care, the LPN will assess, develop care plans, monitor, measure outcomes, report, update professional competence, and participate in client-focused teams with other healthcare providers. Responsibilities also include advocating for clients, communicating needs to healthcare stakeholders, reporting abuse or unsafe conditions, educating residents and families on healthcare issues, teaching use of assistive devices, and raising awareness of community services. Supervision and monitoring involve teaching, delegated acts to healthcare staff, peer monitoring, and preceptorship. Information management includes documentation and producing reports, including Resident Care Plans.
Qualifications: 1. Licensed with the College of Licensed Practical Nurses of Manitoba. 2. Sufficient practical experience. 3. Mentally and physically able to perform duties. 4. Knowledge of long-term care principles. 5. Supervisory ability. 6. Concern for quality care. 7. Ability to work independently. 8. Good communication skills. 9. 1 year in Long Term Care, clinical, supervisory experience; SWAT certification or to be obtained; MDS/computer skills.
Typical Duties and Responsibilities: Assist in developing care standards, supervise staff, perform resident care, maintain records, admit and orient new residents, review care plans, supervise staff, monitor resident condition, manage medications, schedule appointments, monitor supplies and equipment, and adhere to policies and procedures.
