Edmonton Zone Continuing Care offers services across five main areas: Home Living, Supportive Living, Facility Living, Transition Services, and Palliative Care. We seek a Registered Nurse to join our Home Living team, providing professional support in clients' homes of all ages and stages of life.

The RN will collaborate with the healthcare team, assessing needs, delivering therapeutic nursing services, coordinating resources, setting goals, and evaluating outcomes. Responsibilities include case management, direct client care, and acting as a clinical expert and team consultant, especially in triage, clinic support, and community visits involving cardiac, IV therapy, and wound care.

Role Responsibilities

• Provide comprehensive nursing services to patients and families, ensuring safety and well-being.
• Utilize nursing processes, critical thinking, and decision-making skills.
• Educate, counsel, and advocate for patients and families.
• Support holistic, patient-centered care aligned with AHS values.
• Participate in staff development and mentorship.

Qualifications

• Completion of an accredited nursing program.
• Active or eligible registration with the College of Registered Nurses of Alberta (CRNA).
• Current BCLS-HCP certification.
• If inactive for over five years, completion of CRNA Medication Management and NextGenU Health Assessment courses is required.

Experience and Skills

• Minimum two years recent nursing experience or recent completion of a refresher program.
• Ability to work independently, demonstrate sound judgment, and organize effectively.
• Excellent communication and assessment skills.
• Proficiency in computer applications including Microsoft Office and Internet.
• Physical ability to perform nursing tasks, including client transfers and lifting medical supplies.

Preferred Qualifications

• Baccalaureate degree in nursing preferred.
• Palliative, geriatric, or Continuing Care experience.
• Experience with Connect Care, RAI Home Care assessment tools, and case management.
