As the Transitional Case Manager (TCM), you will facilitate seamless transitions for patients from facility settings to post‑acute care. You will verify home health orders, assess care requirements, and ensure continuity of care. Your role includes assessing patients' health literacy, involving patients and families in care planning, providing education to improve outcomes, and promoting self‑management. You will implement rehospitalization reduction initiatives for patients at risk and communicate with healthcare providers throughout the transition. You’ll enjoy the flexibility to work remotely while tackling challenging responsibilities.

Pay is based on several factors including local labor markets, education, work experience, certifications, etc. The salary for this role will range from $48,700 to $87,000 annually based on full‑time employment. In addition to your salary, we offer a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution (subject to eligibility requirements). We comply with all minimum wage laws as applicable.
