Health Here is seeking a Speech Language Pathologist to work with older adults in private homes and senior living communities, delivering personalized, evidence-based therapy aligned with the Geriatric House Calls model.

You will evaluate, plan, and provide therapy to improve wellness, mobility, and independence, with options for in-person and virtual visits, and collaboration with a multidisciplinary team.

Qualifications

• Valid Speech Language Pathology license or eligibility in practice state(s).
• Degree from an accredited SLP program.
• Proficiency in computer use and EMR systems.
• Current CPR certification.

Responsabilités

• Perform evaluations, develop individualized treatment plans, and provide therapy to promote patient wellness, mobility, and independence.
• Deliver one-on-one care in various settings, including virtual care as appropriate.
• Document clinical activities accurately within electronic medical records.
• Collaborate with interdisciplinary teams to support comprehensive patient care.
• Educate patients, caregivers, and staff on safety, function, and aging-in-place strategies.

Role:

Physical Requirements:

Ability to perform moderate physical activities including lifting, repositioning patients, and driving between locations, with necessary use of PPE.
