ChristianaCare is seeking a Clinical Pharmacist to join our team. This full-time role (80 hours biweekly, 8-hour shifts) primarily covers day shifts with evening and weekend/holiday requirements; every 4th weekend and 1-2 holiday shifts per year.

This position supports direct patient care through interdisciplinary rounds, medication therapy management, and collaboration with providers. Experienced pharmacists and residents are encouraged to apply.

Qualifications

• Pharmacist with BS in Pharmacy or PharmD required.
• ASHP residency or equivalent acute care experience preferred (3 years).
• Delaware licensure or eligibility to reciprocate required.

Responsabilités

• Participate in patient care through daily interdisciplinary rounds.
• Manage medication therapies per policies (warfarin, vancomycin, etc.).
• Monitor drug efficacy and safety, adjust dosing as needed.
• Document interventions and provide sign-out to team.
• Educate patients and collaborate with providers.
