Job summary

Everyday Independence seeks a Youth Lead Practitioner to support adolescents and young adults with disabilities across Melton and surrounding areas. You will lead a multidisciplinary team, coordinate therapy packages, and work within the social model to deliver services in homes, schools, and communities.

The role requires a relevant degree and registration, 2+ years of youth experience, strong communication, and a collaborative spirit.

Qualifications

• Minimum 2 years experience working with youth aged 9-25.
• Valid Australian driver’s license.
• NDIS Workers Screening Check.
• Working with Children’s Check.
• Registration in relevant field (Youth Work or Social Work) preferred.

Responsibilities

• Lead the interdisciplinary youth team planning NDIs funded therapy packages.
• Support youth to achieve educational, personal, social, and work goals.
• Deliver therapy in diverse settings (homes, schools, community).
• Document and bill sessions through online system.
• Engage with families and supports to foster ongoing progress.

About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9-25 as they navigate life with a disability, from adolescence through to early adulthood.

As a Youth Lead Practitioner At Everyday Independence, No Two Days Will Be The Same. If You Were Here Last Week, These Are Some Of The Things You May Have Done

• Led the interdisciplinary youth team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
• Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
• Worked within the social model which means you’ll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
• Held meaningful, future-focused conversations with participants and their supports
• Empowered participants and their families to develop habits and routines
• Built meaningful relationships with your participants and their supports (family, carers, friends)
• Documented, and billed for, each session through our online participant management system
• Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
• You’ll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support

To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.

• If you are a Youth Worker, you will have a Bachelor of Master’s degree in Youth Work and YWA membership
• If you are a Social Worker, you will have a Bachelor or Master’s degree in Social Work and AASW membership
