Job summary

Everyday Independence in Albury is seeking a Youth Lead Practitioner to support adolescents and young adults aged 9-25, helping them navigate life with a disability. You will work across settings, bill sessions, collaborate with a multidisciplinary team, and drive outcomes with NDIS funded packages, ensuring person-centred, goals-driven care.

Requires a relevant tertiary qualification and registration, plus at least 2 years of youth experience, strong communication, and a commitment to ongoing

Responsibilities

• Led the interdisciplinary youth team planning a young person’s NDIS funded therapy package
• Acted as main contact for adolescent and young adult participants and their families
• Used assessment and intervention skills to help youth achieve educational, personal, social and work goals
• Delivered therapy in diverse settings (homes, schools, playgrounds, workplaces, community centres)
• Documented and billed for sessions in the online system
• Participated in weekly team meetings to share impact narratives

About The Role

We are looking for Youth Lead Practitioners to support adolescents and young adults aged 9-25 as they navigate life with a disability, from adolescence through to early adulthood.

As a Youth Lead Practitioner At Everyday Independence, No Two Days Will Be The Same. If You Were Here Last Week, These Are Some Of The Things You May Have Done

• Led the interdisciplinary youth team that plans a young person’s NDIS funded therapy package, acting as the main contact for adolescent and young adult participants and their families
• Utilised assessment and intervention skills to support youth participants to achieve their educational, personal, social, and working life goals
• Worked within the social model which means you’ll have worked and delivered therapy in diverse settings, from homes and schools to playgrounds, workplaces and community centres
• Held meaningful, future-focused conversations with participants and their supports
• Empowered participants and their families to develop habits and routines
• Built meaningful relationships with your participants and their supports (family, carers, friends)
• Documented, and billed for, each session through our online participant management system
• Attended the weekly team meeting in the Hub, sharing stories of impact, coffee and laughs
• You’ll have met with your Team Leader and Therapy Mentor to tap into their clinical knowledge and/or support

To work as a Youth Lead Practitioner, you will have a relevant tertiary qualification and registration.

• If you are a Youth Worker, you will have a Bachelor of Master’s degree in Youth Work and YWA membership
• If you are a Social Worker, you will have a Bachelor or Master’s degree in Social Work and AASW membership
