Job summary

The Bridge Youth + Family Services in Victoria is seeking an Early Intervention Youth Worker to support young people aged 12–25 facing mental health challenges, providing tailored support, care coordination and access to integrated services that promote wellbeing and positive life outcomes.

Qualifications

• Experience in outreach and case management models in youth services.
• Ability to implement risk and needs assessments when supporting young people.
• Strong organizational skills and ability to work autonomously and in a team.

Responsibilities

• Provide developmentally appropriate services to vulnerable young people in the local area.
• Undertake assertive outreach to engage at-risk youths and maintain contact with services.
• Utilize multiple modalities including intensive case management and care planning.
• Link young people to health, housing, education, employment and recreation supports.

Education

Diploma level qualification in youth, welfare, mental health, social work or related field
