Job summary

Sirv is seeking supportive professionals to assist young people with complex needs in a nationally operating disability service. You will work with clients, families, and a multidisciplinary team to deliver recovery-oriented, person-centered care that enhances independence and quality of life.

Applicants should have 2+ years in youth, disability, or aged care, with trauma-informed practice. A valid driver’s license, recent police check, First Aid/CPR, and NDIS Screening are required.

Qualifications

• Minimum 2 years’ experience in youth, disability or aged care support with complex client needs.
• Experience with dual diagnosis, trauma history, or psychosocial disabilities.
• Current First Aid & CPR, and NDIS Worker Screening Check.
• Police check within past 12 months and valid driver’s license.

Responsibilities

• Provide individualised, trauma-informed support in line with care plans.
• De-escalate behaviours of concern and respond in crisis situations.
• Assist with daily living activities, community participation, and skill development.
• Support clients in managing mental health symptoms, appointment attendance, and medications.
