Job summary

UnitingCare Health Pty Ltd. is seeking a Wellness Partner for the BlueCare Kingaroy Canowindra Aged Care Facility to deliver engaging wellness activities that enhance residents’ quality of life.

You will work closely with residents, families and the care team to tailor programs, promoting independence, dignity and participation. You will facilitate both group sessions and individual activities, adapt to diverse abilities, and help document wellbeing changes.

Qualifications

• Facilitates group wellness activities and one-on-one sessions.
• Employs person-centred approaches to support wellbeing.
• Works with families and care teams to tailor programs.

Responsibilities

• Deliver engaging wellness programs for residents’ physical, emotional, social and cognitive wellbeing.
• Build relationships with residents and volunteers through meaningful engagement and companionship.
• Assist with wellness assessments, documentation and reporting changes in wellbeing.
• Prepare safe activity spaces and equipment and support a multidisciplinary team.
