Job summary

You Matter is seeking a proactive post holder to lead on a six-bed pre-rehabilitation service for young people affected by ketamine use, ensuring intensive, person-centred support to prepare for residential treatment at RISE. The role focuses on therapeutic relationships, motivation, and addressing health, mental health and social needs within a trauma-informed framework.

Qualifications

• At least 1 year experience with vulnerable adults
• Experience in resettlement, welfare rights, benefits advice, mental health issues, offending, drug and alcohol misuse, harm reduction
• Experience providing intensive support to vulnerable client groups
• Knowledge of safeguarding protocols
• Ability to conduct comprehensive and accurate risk assessments
• Excellent IT, communication, interpersonal, interviewing and assessment skills
• Plan and deliver workshops and training sessions to residents, staff and partner agencies

Responsibilities

• Provide day-to-day support to young people in the six-bed pre-rehabilitation service.
• Act as a named keyworker for allocated service users.
• Build positive, therapeutic relationships to encourage engagement and personal development.
• Complete comprehensive assessments, including risks, needs, strengths, and recovery goals.
• Develop, implement, and review individual support plans and risk management plans.
• Coordinate care with internal and external partners and monitor risks.
• Maintain accurate, confidential case records and progress notes.
• Liaise with external agencies including drug and alcohol services, mental health services, social care, housing providers and education providers.
• Participate in multi-agency safeguarding meetings and case reviews.
• Support access to healthcare, education, training, employment, and community opportunities.
• Promote a trauma-informed, strengths-based, person-centred approach.
