Job summary

Cafs is seeking a Home Based Care Case Coordinator in Ballarat, VIC. This full-time permanent role focuses on coordinating care placements for children and young people in foster and kinship arrangements.

You will work with carers, Child Protection, families, schools and health professionals to support case plan goals and placement stability. The role emphasizes trauma-informed practice, relationship-building, advocacy and collaboration to secure safe, nurturing homes and meaningful family

• Strong relationship-building, case coordination, advocacy and problem-solving skills.
• Commitment to trauma-informed practice and child-centred care.

Responsibilities

• Coordinate and support foster care and kinship care placements in line with approved case plans.
• Maintain regular contact with children and young people, monitoring wellbeing and identifying emerging needs.
• Provide ongoing support, supervision and coaching to carers to strengthen placement stability and carer capacity.
• Work collaboratively with Child Protection and key stakeholders to achieve positive outcomes for children and young people.
• Advocate for children and young people to access education, health, recreational and community-based opportunities.
• Support safe and meaningful family contact arrangements in accordance with case plans.
• Prepare reports, case summaries and professional recommendations as required.
• Maintain accurate and timely case records and documentation.
• Identify, assess and respond to risks in accordance with legislative and organisational requirements.
• Apply trauma-informed and therapeutic approaches, including TBRI, to all aspects of practice.
• Contribute positively to a collaborative and values-based team culture.
