The role spans inpatient and outpatient settings, requiring leadership, education delivery, and active involvement in transplant logistics.

Qualificações

• Registered Nurse (Division 1) with AHPRA registration.
• Post-registration qualifications in relevant clinical area.
• Minimum three years’ clinical experience in transplantation or heart/lung care.
• Ability to work autonomously within an interdisciplinary team.

Responsabilidades

• Provide specialist nursing care and care coordination for heart failure and transplant patients across inpatient and outpatient settings.
• Coordinate patient and family support throughout the transplant journey including work-up, transplantation, discharge and follow-up.
• Collaborate with multidisciplinary teams to optimise patient outcomes and transitions of care.
• Deliver patient and family education, participate in nurse-led clinics and monitor outcomes via PREMs and PROMs.
• Provide clinical leadership and participate in transplant on-call services and logistics.
