• Manage a caseload of older Australians receiving government-funded aged care services in their homes.
• Conduct client assessments and develop personalised Care Plans.
• Monitor and manage client budgets in line with government program requirements.
• Coordinate services to ensure clients receive the right support at the right time.
• Support new clients through onboarding and ongoing service delivery.
• Build strong relationships with clients, families and key stakeholders.
• Work closely with clinical, scheduling and service delivery teams to deliver coordinated care.

Do you have experience supporting older Australians and a passion for helping people navigate their aged care journey?

As a TRANSPAC Care Partner, you will work closely with clients receiving government-funded aged care services in the North Sydney region. You'll build meaningful relationships with clients, assess their needs, coordinate services and advocate for positive outcomes that help them remain safe, independent and connected in their own homes.

This is a 6-month fixed term position, combining community-based client visits with the flexibility to work autonomously from home.

• Full-time, 6-month fixed-term position, 75 hours per fortnight, with standard working hours of 8:30am to 5:00pm, Monday to Friday.
