Job summary

Teachers Health Fund is seeking an experienced Registered Nurse Care Coordinator to join our Healthcare Services team. This role focuses on specialist telephone-based care coordination, helping members navigate their health needs and build personalised care plans.

You will provide health coaching and connect members with the right services and support. The position offers a hybrid working model and opportunities to make a genuine impact on members' health.

Qualifications

• Current unrestricted AHPRA registration as a Registered Nurse.
• 5+ years' full-time equivalent experience as a Registered Nurse.
• Knowledge of primary and community care environments.
• Strong verbal and written communication skills.
• Health coaching knowledge and capability.
• Strong computer literacy and proficiency with the Microsoft Office suite.

Responsibilities

• Identify and engage eligible members through referrals, enquiries and outreach programs.
• Support the assessment, development and monitoring of personalised care plans.
• Coordinate care with members, healthcare providers, service providers and broader care teams.
• Provide health education, guidance and support within your clinical scope of practice.
• Maintain accurate clinical records and contribute to quality care outcomes.
• Participate in clinical audits, professional development and continuous improvement initiatives.
• Contribute to a positive, safe and member-focused team culture.
