HammondCare's Dementia Support Australia (DSA) invites a Team Leader to guide a multidisciplinary staff across Daw Park, SA, Darwin, NT and other Australian offices. You will provide professional leadership, oversee case management and ensure KPI targets are met while maintaining a patient-centred approach.

Qualifications

• Tertiary qualifications in Nursing/ Allied Health or health-related degree.
• Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management.
• Ability to manage and support a team of skilled professionals across multiple sites and locations.
• Displays strong customer service ethic and attitude.
• A current Australian driver’s licence and willingness to travel intra and interstate.

Responsibilities

• Provide professional and clinical leadership for team members across multiple sites.
• Ensure timely allocation of incoming referrals to the appropriate consultant in alignment with KPI targets.
• Oversee office environments including safety, routine maintenance and functioning equipment and ICT.
• Oversee fleet vehicle usage, tracking and reporting.
• Approve brokerage and travel in line with financial delegation of HammondCare policy.
• Maintain wellbeing and safety of team members and manage incidents with leadership.
• Report to Regional Manager on staff performance, service quality and operations.
