Job summary

Hogan Care in Greater Melbourne, Geelong, is seeking compassionate Support Workers to help clients with day-to-day living in a range of settings. You will provide person-centred support to assist clients in achieving their personal goals and living independently.

Ideal candidates will have strong communication and interpersonal skills, the ability to work independently or in a team, and relevant qualifications such as Certificate III in Individual Support or similar.

Responsibilities

• Support clients in achieving personal goals and independence.
• Maintain accurate records and progress notes.
• Follow individualized care plans and report changes.
• Respond to emergencies or urgent situations as required.
• Assist with personal care (showering, dressing, grooming, toileting).
• Support daily living tasks including meals, cleaning, and laundry.
• Help with mobility, transfers, and use of equipment.
• Provide companionship and assist community participation (shopping, outings, appointments).
• Promote independence, dignity and wellbeing.
• Comply with policies, safety and quality standards.
• Assist with community access such as taking participants to activities or appointments.

Key Responsibilities:

• Support clients in achieving their personal goals and promoting their independence
• Maintain accurate records and reports of care provided, including progress notes
• Follow individualized care plans and report changes, and keep accurate records
• Respond to emergencies or urgent situations as required
• Assist with personal care (showering, dressing, grooming, toileting)
• Support daily living tasks including meals, cleaning, and laundry.
• Help with mobility, transfers, and use of equipment.
• Provide companionship and support community participation (shopping, outings, appointments)
• Promote independence, dignity, and wellbeing
• Comply with policies, safety, and quality standards
• Assist with community access support such as taking participants to outdoor activities, clubs and/or appointments.

Requirements:

• A caring and empathetic nature, with a genuine desire to make a difference in the lives of others
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Previous experience as a support worker or similar role (preferred, but not essential)
• Relevant qualifications such as Certificate III in Individual Support, Aged Care, Disability, or similar (preferred but not essential)
• Current First Aid and CPR certification
• A valid driver’s license and access to a reliable vehicle (if required for the role)
• Desirable: personal experience with a disability in family/ friends
• Desirable: experience with supporting epilepsy
• Availability to work flexible hours, including active overnights, weekends, and public holiday
